Appointments are generally available Monday-Friday, 8 am- 5 pm. Keep in mind that The Dermatology & Laser Center has three convenient locations. Be sure to check your appointment reminder for the scheduled location.

Required Appointment Information

Please have the following information available when scheduling your appointment and bring it with you to the appointment.

• Patient’s Name
• Patient’s Date of Birth
• Insurance Card (Subscriber or Insured’s Information)
• Pharmacy Name and Phone Number
• Bring Current List of Medications

To decrease the time you have to spend in the waiting room, please take a few minutes now to print and fill out the forms required to assist us.

Cancellations and Rescheduling Policy

Please contact us at your earliest convenience if you are unable to make your scheduled appointment. This courtesy will allow our office to make the appointment available for other patients. We thank you in advance for your cooperation.
